Preliminary Information and Questionnaire For Prospective Painting From the
Source Trainees.

Dear Prospective PFTS Trainees, Artists, and Healers,

Thank you for your interest in PFTS Trainings. You are one of 150 people
with various goals, backgrounds, aspirations and needs, who have requested
information about PFTS trainings. Some of you may want to teach PFTS in your
Geographic area, some may want to augment PFTS techniques with your current
work or profession, and some may want to use PFTS as a personal exploration
of your larger work purpose.  Below are: preliminary information and a
preliminary questionnaire.

Information Update:  As of now, June, 2001, the first 2 year Intensive
training of 14 PFTS Practitioners (mainly people with no Mental Health
Degree), is entering its 2nd year, to be completed July 2002. These
graduates will be legally entitled to call themselves PFTS Art Medicine
Practitioners and use the PFTS logo.  The complete cost including room and
board for 6 ten-day sessions and travel averages about 16 thousand dollars.
Now, half way into this training I realize that while very magical and
gratifying,
it is also a major energy challenge for me that I am not sure I want to
repeat. I also realize in terms of your needs, that this kind of passion and
commitment of time and money for PFTS Training is not for everyone.
There are alternatives that can be less expensive, time consuming and
intense.  The program could be structured in a more introductory format,
which might work for those with no mental health degree and/or better fit
your professional needs. Many of these shorter, more condensed formats,
would
understandably not entitle you to legally advertise your self as a PFTS
Practitioner, (it would be like an "audit" status in a "graded course"), but
you would have some essential knowledge and skills. And in this case you
could say on your advertisements that you have been influenced by
or use techniques from Aviva Gold, author of PFTS. There could also be a
one-year course for those with a mental health degree who would be eligible
for certificate. Also CEU's are now available for mental health
professionals. What ever your status, it is necessary to attend a workshop
to be considered for acceptance.

Questionnaire: In order to better design the PFTS Training programs for you

consideration in any training.

2. HAVE YOU EVER TAKEN A PFTS WORKSHOP? IF SO, WHAT WAS THE DATE, LOCATION
AND LENGTH OF TIME (WEEK-END, 5 DAYS?)? HOW MAY I REMEMBER YOU?

3. DO YOU HAVE A MASTERS DEGREE IN THE MENTAL HEALTH PROFESSION?
IF YES, WOULD YOU BE INTERESTED IN A 1-YEAR CERTIFICATION PROGRAM? AN AUDIT

SPEND APPROXIMATELY 16 THOUSAND DOLLARS, COME TO 6, TEN DAY SESSIONS OVER A
2 YEAR PERIOD AND DO READING, LEAD WORKSHOPS AND OTHER ASSIGNMENTS?

7. CONSIDERING YOUR GOALS AND USE OF THE TRAINING, WOULD YOU ACCEPT A
SHORTER MORE INTRODUCTORY TRAINING (LEARNING BASIC TECHNIQUES) THAT WOULD BE
